Personal History Form- IMM5669 Schedule A - CIC

Hi everyone- I wanted to confirm if this is the right personal history form or is it an old one? I tried to find it online from CIC's website under Express Entry, but it did not provide specific links. I am yet to apply so don't know have a personalized checklist.

I also had a question about address history. I reside in Canada since September 2015, so for previous address i.e. from my home country, should I write the end date to September 2015 or ongoing as I go there whenever I take vacations? Also, I have been living there since by 5th birthday, so should I mention the starting date as my 5th birthday or 18th birthday. Does it make any difference?

Can someone please share their experience? Thank you!!!

Also, guys I found the new version's link, I think (see below), and have started filling it up but it won't validate for some reason, I don't know what am I doing wrong? It puts a red box on my 2015 date for some reason

I cannot confirm the document. Someone else can, hopefully.

Address history is only for the last 10 years and there should NOT be any gaps.

Short trips are not mentioned in the address history but in the travel history. In the travel history, there is a column for address of that trip.

Once you receive ITA, there are stationary questions, personal history form, travel history form, and address history form. Once you populate all these and confirm, you will get personalized ITA checklist.

Thank you for the reply. I was just filling out the details to make sure I get the details ready. I see, what personal history form is this? Is it an online version or something? So does this mean, after one gets ITA, they have to submit details online before submitting the PR application?
