Hi,

I have been here as a care giver for 2 years and planning to apply for PR soon.

In the background information, under - have you ever been denied entry into Canada or any other country.... I am not sure how to answer this question, my situation is as follows:

I had applied for US visa and attended an interview in June 2018, my application was rejected. I am assuming because my work permit was expiring end of June. Once my work permit was renewed, I re-applied for US visa and this time my application was approved and US visa issued. What should be the answer to the above question?

1. Should I simply answer NO

or

2. Should I say visa was denied on X date and subsequently approved on Y date?

Please advice.

This sounds just like a visa refusal which should be declared in another question. Being denied entry is different: you actually landed in the country (even after a visa was issued) but you were not allowed to enter.
