when we receive ITA then which time is better to go for medical ? are they requested for medical?
or it will upfront exam?
 
Prefer to have it around a week prior to the date you decide to submit the documents.. as visa that would be stamped after PPR would usually have validity till one year from your medical date

Post ITA .., for Medical Appointment., do i have to contact the Listed Physicians directly or some IRCC side appointment has to be given.?
I'm confused for this. Help

Just contact them directly and get an appointment. IRCC is not involved in this as your results are directly uploaded by approved physicians.

After completion of Medical exam they give us a report like a paper as i heard. so we should upload as well?
 
yes, That is the only thing you need to upload in Medicals placeholder and IRCC will be able to access your medical results based on that paper.
