I am starting this thread for December 2018 applicants and i am one of them :)
Can the experts/seniors advise on the below questions from the application form

Q06 - If no name changes , should we just write our name select Name at birth ?
Q11 - I know we should give employers address , but just wondering about the name of contact , phone number , email address columns
Q12 - should we tick Yes for both Required to File and Taxes files ? my edibility period is from 2013 DEC-2018 DEC

Q06- if you didn't change your name select 'Name at birth'
Q11- just write either the name of your HR manager or your boss name, phone number and email address.
Q12- tick what is relevant for you. If you were required to file the taxes tick 'Yes' for both 'Required to File' as well as 'Taxes filed'
If you were for example a student that time and you were not required to file the taxes but you filed it, just tick 'No' to 'Required to file' and 'Yes' to 'Taxes filed'. I hope it's clear to you now.

to your first question - select Yes to Required to File and Yes to taxes filed.
Yes, if you lived about 120 days in your home country then provide your address there.

December 2018 applicant here - just started the process of completing the application form. Some questions right off the bat:

12b) Tell us when you filed your income taxes in the last five (5) years using the chart below

Question 1: I filed for 2017, 2016, 2015, and 2014 (all the years I have been in Canada) - 2013 I was still in Korea. Do I need to include "2013" (or any non-Canadian tax years) in the chart?

Question 2: How do I know if I was "required to file"? I filed regardless, but unsure to know if it was actually mandated. I am assuming this is meant for folks who didn't work thus didn't have income tax to file?

13) Have you ever had immigration or citizenship status in any other country other than Canada, including your birth country?

Question 3: I put down South Korea as my birth country and then I put "obtained" as my birth year and birth date. Other than that I have not had any other statuses in other countries beyond being a PR here in Canada. I am assuming travel visas and/or stamps for the purpose of vacation does not count as an "immigration status"?

14a) Have you held travel documents and or passports during your eligibility period?

Question 4: "Travel documents" seems vague to me. I traveled to the US and South Korea for vacation / visit purposes - do they want to know if I have "travel documents" like plane tickets or stamped entry visas?

15) Language evidence

Question 5: I completed the CELPIP - LS (Listening and Speaking) and exceeded their minimum "level 4" requirements. Is my certification good enough language evidence?

[DN] I believe, you don't need to provide Korea tax assessment certificate.

[DN] Since you have lived here, you need to file tax returns - it does not depend on whether you have income or not. Not required is for those who did not live in Canada probably for whole year.

[DN] Your assumption is correct.

[DN] I think 14a is asking if you hold passport during your eligibility period - this question is meant for others who are having complicated history.

[DN] I am not sure - I will be using same IELTS certificate which I used for PR application.

Just further clarification on the Q12 chart for taxes: so if I was in Korea during 2013 (I landed as a PR in Sep 2014), so I only have 4 taxable years in Canada thus far... but for 2013 I would just fill that last line in the chart with "2013" and put "no" and "no" to Required to File and Taxes Filed?

I think they're similar. Not entirely sure what the Canadian passport specs are, but you can find them online if you look it up via Google.
