Is it important to maintain the minimum income threshold until the outcome of PR for parents?

Has there been any instance where the PR has been declined because the Sponsor's current year (the year application outcome was decided) income was less than the required income?

Yes there have been many cases where apps have been denied due to this.

If you are referring to a parents application, the visa officer typically does a final income check before approval, that the sponsor has continued meeting LICO for later tax years after submitting the app. If sponsor dipped below LICO in any later year, app would be denied.
