Fill out the information to the best of your ability but make sure to include all the names of your family members that are asked for.

Thanks.
Will CIC ask for any supporting evidence or document against those names at any stage in this application process because we literally don’t have anything to provide as we are not in contact with parents’ siblings by any means since ages. Are they going to verify those names, addresses, email and other related information ??

Hi canuck78,

A few things if you are aware of -
1. If I include all the names of parents’ siblings and put rest all fields to NA would that be ok?
2. If I put NA in everything would that be ok?
3. If I include a couple names and not all names and put rest all fields to NA would that be ok?

Please assist.

Here's the very first sentence on the top of the form.

The principal applicant, their spouse or common-law partner (if applicable), and all dependent children 18 years of age or older must complete their own copy of this form.

That's why I said, if you have a child who is 18+ but not a dependent, he/she is not required to fill out this form. This is for dependent children only. Accompanying or non-accompanying... that's a different story.
Please quote me if I’m wrong.
This for is for -
1. You - principal applicant -
Example - either your mother of father, one of them will be principal applicant, up to you to choose

2. Your spouse or common-law partner -
Example - Once you have set principal applicant, this will be their spouse or common-law partner

3. Your dependent children aged 18 years or older (whether accompanying you to Canada or not) -
refers to the principal applicant dependent children(s) 18 or above, regardless they will join you in Canada or not just in relation to this application. I think the main purpose is to know how big is your family and future references only.

Filling out application forms for both my parents. Do I have to submit 2 separate forms for my Mother and Father? If using one, one becomes principle applicant, where do we enter the 2nd applicant?

Your question is not stupid, and I can reply you from personal experience .
Last year when we submitted my parents in law application , there were obvious differences in paper size and print quality of the pages that they sent back to us and the ones we printed here. We sent them to IRCC like that and received AOR a few days after. We are now post sponsorship approval and awaiting IME and PCC requests.

My guess is that as long as the forms are filled correctly , no spaces, all signed and dated, all supporting documents intact, they don't really care about paper size.

Hope this helps. Cheers
Hi,
Does it mean that if they take the print out of the completed version of the form of the signature page only and sign that and then scan and email back so that the sponsor can take the print out of that scanned copy and forward it to CIC. Would that acceptable ?

Can you please let me know how much it will take for visa to be granted?

I am noticing that a lot of sponsors realized after sending the application that they missed a form or did some mistake. I will suggest everyone to take their time in reading the application guide and then the application forms. There is still some time, rushing to submit application and missing a form or information could result in a lot of delay in processing of applications. Good luck to all fellow sponsors.

@canuck_in_uk: Considering the the Guide 5772 says the following, do you still think adding a valid date to get the form to validate successfully is a good idea? As far as I understand, this form does not print a barcode page even when validated successfully.

Note: If you do not know the complete date of birth or if the date is in the 1800s, please use a “*” (star sign/asterisk) to fill in the spaces for the unknown year, month or day. The missing information can be added by hand however the form cannot be validated.

Hi,

I’m not able to validate a form if I put in the year in 1900s with no information about month or date as not known? Also, parents’ parents were born before separation of India and Pakistan. I’m still thinking what to put in as country of birth?
Asterisk doesn’t work with validation

Q1:
I have a family of three thats me, my spouse and a daugther. I am sponsoring my both parents that mom and dad. My younger bother also lives with my parents but i am not adding him in the form as a dependent.
I am the only sponsor. so in the form i am adding head count as

2. You = 1 (myself)
3. Current undertaking = 2 (mom and dad)
5. Spouse = 1
6. Every other family member = 1 (daughter)

So total = 5 people.

Is it right?

Q2: If i am providing my SIN number with the application in the section 8. Then i don't need to fill and send IMM5728.
and with the application package i don't need to send my NOAs or employee salary letters etc.?

Q3. what finiancial documents should i send with the application.
back statements, affidavits, employement salary letter, proof of assets etc?

Q4. is there any requirement for sustaining minimum balace in my account to show? if required?

Q5: The documents so far i am adding witht he application package are
  1. My PR card (sponsor)
  2. My Birth certificate
  3. My passport
  4. My marriage certificate (although i think i don't need it but still adding it)
  5. My daughter passport (to show the proof of my dependent )
  6. My parents passports
  7. My parents marriage certificate (not in english) + english translation + affidavid
  8. 2 photos of my parents
are there any other docuemnts that i am missing and should add to the application package?[/QUOTE

Hi,

Does all the documents required are only ‘photocopies’ of the original unless not in English and French? Is that correct ?

Adding to it, only photocopies of the original and do not require certified or notarised unless documents are not in English and French. Is that correct ?

Your questions have already been answered. Stop asking the same thing and expecting a different answer. ALL of the family members MUST be listed. Failure to do so is immigration fraud. This can result in your parents' app being refused and a ban from Canada.

Again, already answered. Signatures must be ORIGINAL, not scanned.

Hi,

I have questions while including traveling history in form 5669, Personal History. My father is still working and my mother is retired, they have visited me in the past.

My questions are:
  • How should I mange the dates to include the travels? For example, my father is holding the same position since 2013, so he is employed since 2013 and it is ongoing activity. He visited Canada in 2016 and that visit does not stop his employment in his country, he was still employed. When I tried to include employment since 2013 and a visit in 2016, the validation failed.
  • What activity type use during those visits? He is still employed in his country but he is not employed in Canada. I am worried that if I put employment as activity during the visit it can be interpreted as if he was employed in the country that he was visiting, but if I use another activity type it will look as his employment was interrupted, what it is not true either.
The same apply for my mother, even if she is retired, that does not stop because she is visiting me but her status during those visits is not retired in Canada.

The example in the guide does not help because the activity is unemployed and it could be be the same in the visiting country and in the country where they are living.

Any help will be appreciated.

Thank you

I can tell what i did for this form. For the employment history i incuded only employment history without any interruptions for visits during vacations. I entered info re addresses abroad for the periods of their visits. I.e. addess in home counrty from May 2015 to June 2016, then address in Canada from July 2016 to Aug 2016 (my address as my parents stayed with me when they visited), then agai address in home country from Sept 2016 to July 2017 etc. Because their employment was not interrupted during their visits.

Assuming you worked for a company XYZ, and assuming family size 5 with gross income 90k, I would fill the form year wise, see the below example:

Thanks @gst8486 for the useful info, much appreciated.
However, in the interest of being careful of filling out this section, I want to ask this question again (apologies if it has been answered previously):
Should the "gross income/benefits for period" column include line 150 from Notice of Assessment (NOA) for that period? Or the YTD amount from the last paycheque from that year?
The reason I am asking is that at least in my case, these amounts are not equivalent, since the NOA includes income from other sources (like dividends, stock trading etc.) that are not tracked in the paycheque.
Thanks.
