I need to know the cost of Medical for super visa in Karachi for both (Mom and Dad). Also can we get the medical insurance from Pakistan or is it necessary to get it from Canada

The initial cost for the medical exam is approximately 12,000 PKR. However, if additional tests are required, then the fee will vary depending on what additional tests need be done. For example, if they need to do an ECG, you will have to pay extra.

Regarding getting insurance, you have to get it from a Canadian Insurance company. You cannot get insurance from a Pakistani company. I would recommend TravelShield as the allow you to pay monthly and have reasonable rates.

Yes you can - you'll need to fill out the representative form.

I need some advice to apply for supervisa. I have filled in the basic forms and got documents checklist. Now I have few questions, my parents marriage certificate is in native language so where do I get it translated secondly for purpose of travel what document I should upload as they are planing to stay around 6 months.

You can get it translated from notary public, get it that attested/stamped by them. You'll need to upload both, the attested translated version and also the copy of marriage certificate in native language.
There's not really a document for purpose of travel - you can upload the travel itinerary

For travel itinerary, their return tickets are enough to upload?
Also one more thing do I need to upload their national identity cards although it’s not required as a document but just in case if they ask later?

their national IDs are not needed and as for itinerary - please don't buy tickets as of now - you can get provisional tickets from travel agencies.
