Unfortunately there is no such thing such as immigrating for medical purposes yet not living in Canada. There may be some charitable organizations that could provide treatment but those usually involve surgery. If you try to immigrate via economic pathways (express entry) there is a requirement that you will not place excessive demands on the healthcare system so please research whether you would qualify. If you want to come to Canada as a refugee - you must either obtain a TRV (tourist visa) and then claim refugee status when you arrive in Canada. To be approved for a TRV, you must show you have strong ties to your home country and have no plans on remaining in Canada long term.

Or you must move out of your country, be recognized as a refugee by UNHCR in the new country you move to and then be selected for resettlement to Canada.
