Sounds pretty thorough.

Do I have to physically take with me my entire medical notes???? Seems crazy but otherwise people could simply deny conditions!

People could deny conditions yes. But CSIS will be on their back from the moment they set foot on canadian soil. Dont believe all of that confidentiality stuff, they will find many ways to find people who lie.
 
I would hope so but it's not a difficult thing to do... just don't say. Unless physically obvious.

I'll get a copy of all my medical notes to take as per standard procedure I'm guessing. What do I ask for? My entire file? Last 10/20years?I'm assuming not from birth!

Just things related to a current condition that might cause you to be medically inadmissible based on either danger to public health or causing excessive demand on health and social services.
 
Hi.. is it necessary to reveal to the doctor about drinking/smoking/physical relationship during a medical examination?

Not unless the doctor specifically ask for it. There is an hiv test so sometimes in that they ask you about your sexual life. The questionnaire doesn't have these questions, but rather questions on any prior ailments, medicines currently being taken etc.
