Can you help clarify what the difference between these two forms is?

If I am the sponsor and want to call to inquire or get information on my husbands behalf which do I submit?

you can use either

however, the use of rep form will allow you to deal with the application on your husbands behalf if any changes need to be made or if CIC requests more information/documents from you.
