Use of IMM5406 - who needs to sign

Hi there! Confusion about filling the form of using a representative. If husband and kid (6 y.o ) are dependants on a permanent residency demand, do all people concerned in this demand need to sign a IMM5406? Or just the principal applicant? Thanks!!

It's actually 5476. All the people concerned should sign a separate form except for dependent children under 18 years of age.

By signing, you authorize IRCC to complete your request for yourself and your dependent children under 18 years of age.
