Do we have to upload the Declaration Form (Surrender of RationCard/VoterID) to the OCI Online Application?

Yes, you have to upload as per BLS office Regina. I combined Surrender Certificate, and declaration form together and uploaded.

All the best. I think the embassy won't be bothered about it. However, BLS tries to earn some extra taking advantage of the confusions in the process. I will wait to hear back from you.

I was able to submit my OCI application but with 1 small issue. I left both the fields "National Identity Number" and "Previous Passport Number" Blank. They didn't say anything about it.
But they tried to sell their Premium Lounge service by pointing a mistake towards my "Occupation"
I listed Occupation as "Professional" and submitted Article of Incorporation documents as I am self-employed. But they argued it should be "Business" even though I am not doing any Business.
Since it was not a busy day, I quickly filled out a New application myself in the parking lot by making that change and took a printout and entered BLS again as an walk-in and waited for 30 mins and was allowed inside.
I also suggest uploading the "Mandatory Declaration Form" to OCI Online Portal, even though it is not mentioned in the Checklist. I didn't do it the first time, so I have uploaded it for the new application.

Its straightforward.

For Voter id: Search your name in current electoral roll to find local EC/Election office where you can submit Form 7 with voter ID (they even give you paper form if they have in lot) Search using EPIC or manually using second option, Search by Details or third option Search by mobile#: https://electoralsearch.eci.gov.in/

For Ration Card: It depends on the state. In my state, its online like you can go to https://rcms.mahafood.gov.in/indexv.aspx and submit the form at local office manually on a paper for cancellation with a copy of existing Ration Card, so it reflects in online records, a couple of trips (if officer is unavailable) and it gets done. You need to cancel/take out your name from the existing members, unless all members have acquired foreign nationality, then cancel whole Ration Card itself.

Do you have a voter id and its not listed in local electoral roll?

The voter ID that I have was the one given to me during 2008. The voter ID is not now the EPIC number. During 2016 I voted using the voter slip given to me at my residence which I don't have any more.

If you voted in 2016, your voter ID already exists — you just might not have the details to confirm it. The electoral roll is available at your local corporator's office, and they can help you retrieve the information.

You're on the correct website. The application you fill on OCI site needs to be submitted through BLS, so tracking happens at two levels — the OCI portal for processing status, and BLS for updates on when consulate received your application and when you will receive OCI card in hand. There's no separate registration required for BLS.

Yes, but he's 4 months old when I applied. You can take thumb impression on a plain paper and upload it. Also, paste the cutting of same in the application form.
It worked for us.
