Got my ITA recently. I did an immigration medical 3 years ago for my study permit. Since then, I’ve only left Canada once — 1 month in India earlier this year.

If no new medical is needed, I can submit my PR this month using my current PCC (which expires end of this month). But if a new medical is required, I won’t have enough time — and I’ll need to get a new PCC too.

So, do I need to redo the medical?

You won't have to redo the medicals. Submit the report/information sheet from the 3 years old medicals which has the UMI/IME number. You can also submit a letter with the UMI number or if you forgot, you can still write a letter stating that you did your medicals for study permit that is 3 years old.

In any case, you do not need an Upfront medical exam to submit PR. IRCC will request you to do medicals if necessary AFTER you submit application.
I do not know how travel to India affects PCC. But if you didn't travel to India after getting PCC, IRCC accepts it even if it has a validity date. Suppose you never travel to India after getting PCC, you can even submit the same after 2 years.
