I'm resident in Canada on work permit along with my wife and minor child (she is 17). When we first arrived 3.5 years ago, my child received study permit. On extension, last year she received vistor record which is valid till Nov-26. She is entering university this year.

1. Which form needs to be used to apply for study permit?
2. If its IMM 5709, then which applications options (such as apply for new or extend study permit; Restore status etc.) should be ticked?
3. What is her current status? Is it visitor?
4. What was her status at time of arrival? Was it student?

I'm unable to submit form and IRCC website is giving an error.

Assume your child already has PAL.
 
When she entered first time, she got study permit. On extension from with in Canada, she got visitor record. Her visitor record says she can study at primary, secondary school.
So she entered as a student and current status is visitor. It is a new study permit since she is a visitor. She cannot study until permit is approved.
