Is this the form where we say we gave our voter id and ration card. I got confused because there was another one declaration form as last page in OCI application form.

The form which you are referring to is this one ?

"With reference to my OCI application file reference number


…………………………, I hereby declare that I have surrendered my Voter ID


and Ration Card to the concerned Indian authorities in India after acquiring


Canadian citizenship or other foreign nationality.


If later on, I am found to have the possession of either of these


documents, i.e. Indian Voter ID card and Ration Card, I will alone be held

If only one parent is canadian citizen , it is enough for them alone to sign ?
this is the correct one. The child does not have to thumbprint this if he/she was born in Canada and was never an Indian Citizen. However, if you and your wife were once Indian Citizens, you both have to prepare separate Declarations for your own selves.
 
I know this is for OCI. Just wanted to check if anyone applied for surrender certificate around Apr 7 in BLS Toronto and got the certificate?

They said 2 weeks. It's been 2 weeks already so stressed.
 
In my case only 1 parent is Canadian citizen so I am not 100% sure if both parents should sign on 1 declaration. But I would suggest to sign separate declarations as initially the declaration for Vancouver consulate was asking to write the ration card and voter id numbers to be written on it, so naturally that would be 2 separate declarations.

Also I don't know why, but BLS asked us to handwrite the entire declaration. So better both parents print>sign>scan>upload and write>sign>scan>upload

I applied for my minor at BLS and both parents just signed 1 documents. I didn't provide two separate copy for voter id declaration.

In my case, both parents Canadian Citizen. For Voter ID/Ration Card Declaration, Only Canadian Citizen Patents needs to sign. For Indian Citizen, you can still have Voter ID/Ration Card.

Yes. Which Jurisdiction did you apply to? Also, do you remember what did you mention in Relationship to root India?

Thanks for your reply. So basically in my case I will just need one declaration for myself which I will need to sign and no other declaration needed for my wife and minor child right? I think they also have different format for declaration in Vancouver and in Toronto.
