Hi everyone!

I have received the same letter. When I filled it out, all my duties (which I copied from my job description) seemed to have taken up all the space and some of the information couldn't be seen anymore. It said if the information does not fit, provide an addendum. Has anyone tried to do an addendum? If so, does anyone have a template of how to do it?
