My wife want to sponsor me for PR, I have 3 children from my previous marriage. Can i fill IMM 5481 and it to my application. Please who is to fill the form?

How old are children? Do you have full custody? Each child must do the medical. Add application for each child.

Go through the checklist again carefully - I don't believe the 5481 form is required. If it is, then the sponsor fills it out.

Then list them as non accompanying, otherwise you might not be able to sponsor them in the future.

I'm not going to fill the forms out for you. I'm saying you need to read the instructions and checklist carefully. Most do not need to fill out that form.
