Ontario Health card for PR PGP

Hello

I HV been issued eCOPR for virtual landing on 15th September.

Can I apply Ontario Health now or I HV to wait for some days?

And

Kindly help me to know list of documents required for Issue of Ontario Health card.

I HV only eCOPR and Passport.
I do not have Canadian driving license.

What other 3rd document is required.

Will the parents be remaining in Ontario for the first 5 out of 6 months after getting the card, the 6 out of the next 12 months after getting OHIP and be making Ontario their permanent home?
