Where do we indicate the NOC code ??

Where in the application did we actually indicate our NOC code ?

I did see some occupations, but I never saw an option to indicate a numerical NOC code.

And what if a combination of two NOCs ?

and if we don't indicate an NOC number in the reference letter, how will they categorize the application ? by the CIO or by the actual visa processing officer ?

NOC codes are in the Schedule 8 (Canadian Experience Class) form under "Your Work Experience In Canada", alongside each job you are claiming toward your 12 months work experience.

It is the processing officer, not the CIO, that decides whether your job duties, etc, correspond to the claimed NOC code.
