What is an affidavit for document translation?

The PR Card application guide states that if we use a certified translator for our document translation, we do not need to supply an affidavit. So if I get my friend or colleague who are non-certified translator to do the translation, I need to supply an affidavit.

What is an affidavit? What are the wordings in it? Can anyone just write this affidavit or does it needs a lawyer/notary?
