Have they removed IMM5257B from the documents list? Where do I fill in my travel history details? On the online portal there is no form against TRAVEL HISTORY. How can I provide them with the details?

There is no form for that. You have an online folder "Travel history" and you have to upload a document with scanned copies of your visas, entry/exit stamps. On the first page of this document you can list all the visited countries and the dates of the trips. That s it.
