VCM requested additional requirements, including IMM5645E

Hello everyone,

I've received emails last Feb to do our medicals and submit the following requirements, (Woohoo!):

1. IMM5669E
2. IMM5645E
3. IMM5562E
4. Latest Police Certificates
5. Passport copies
6. Updated information chart
7. RPRF payment

My question is, i'm applying for permanent residency (FSW1), and i believe IMM5645 is applicable for temporary visa application only, should i fill this up and send the same or reply via email for clarification?

My original application has the IMM5406 (which i think is more applicable), should i send an updated imm5406 instead?

I dont think you should go contrary to their instructions, I just searched for IMM5645E and found it to be the sameone I had filled at the time of application. Perhaps they have recoded the forms.

Hi Mabertia,

Thank you, 5645 and 5406 is almost the same except 5645 is for temporary resident visa applicants. However, you're right not to contradict their instructions.
