Surrender Indian Passport

i want to apply for OCI but I need to surrender my passport. I am wondering if someone can help me as my case is a little confusing.

I moved to the UK from India in 1980
I got my Britain citizenship in 1986
Added my last name and started to use Kaur as a middle name.
I renewed my Indian passport in 1987 I know I shouldn’t have
Moved to Canada in 1992
Got Canadian citizenship in 1996
What documents do I need to submit?
No help at all on the website

I don’t know if I am missing it but I don’t read anything about previous citizenship besides Canadian

I suggest you just go to the nearest BLS center and talk to them. They will take you through the process like KRP mentioned if yours is deem a special case.
