Hello there,

Sorry for the sudden request but I am a little desperate now as I now have only 10 days left after receiving a request letter from the manila visa office asking for an APPENDIX A (not the document checklist) which was not attached to the email that was sent to me. I've emailed the manilimmigration email to request this file but I have not gotten anything from them at all except for those automated replies... I would like to ask if there is anyone who received the same request letter and would be able to tell me if its a generalized or personalized (based of the UCI no. or has specific identifiers) APPENDIX A form.

If it is a generalized (blank) form, I would like to ask if I can get a copy of the appendix A attachment?

Hi, did they ask you to submit the Appendix A with your passport?

This is what the letter said - no APPENDIX A attached though.

In order to continue processing your application, we require the following information and/or

documentation:

 Fully Completed Appendix A (attached) for you and each of your family members, whether

accompanying you to Canada or not. Passport details are not required for any family member who

is not accompanying you to Canada.

 Your personal history from January 2004 to October 2009 (see Completed IMM 5669 - Schedule

A: Background/Declaration form--include jobs held, periods of unemployment, periods of study

and any other extended use of time, such as time spent travelling in search of a country of refuge,

stays in hospital, prisons or other places of confinement, as well as periods spent at home as a

homemaker. PLEASE DO NOT LEAVE GAPS, AND USE MONTH-YEAR FORMAT.)

This documentation is reasonably required to complete the assessment of your application. If we do not

receive all of the above documentation from you WITHIN FORTY-FIVE (45 DAYS) of the date of

this letter, a final review of your application will be completed based on the information available on file.

Please note that this may result in a refusal of your application.

Sa nga naka receive na po ng visa dito through DHL, anu po ang address at name na ginamit nila na makikita sa envelope ng package from them?

ano po timeline nyo?

August 7, 2014 po pala passport, appendix A and PRfee requested. August 8, 2014 narecieved po sa CEM. Until now wala po update kahit ano sa ecas. Wala po kahit ano delivery. Ayaw ko na nga po magtrabaho next sem, pakiramdam ko dadating na po talaga passport at hindi na aabot ng November.

What if I am a TFW here in Alberta and my soon tobe wife is a PR in Vancouver..Then we applied for my PR am I eligible aswell to apply for a open work permit? Thanks in advance?
