Single Parent applying Visit visa with minor in UAE

Hi. A friend of mine will come along with me to Canada for tourist but while she is filling-out the form for minor, there are some unclear things. Please can anyone assist us.

For Single Parent:

02. Family Information IMM5645E - ok
03. Itinerary, Travel - ok
04. Proof of Income
05. Purpose of Travel - Other -ok
06. Proof of Means of Financial Support - ok
07. Passport or Travel document - ok
08. Digital Photo - ok
09. Employment Reference Letter - ok
10. Travel History - ok

11. Use of a Family Member Representative for Online Application [IMM5713E]
Section A-1+2 Name/Birthdate of the Single Parent
Section A-3 Name only of the minor accompanying
Section B Blank as the accompanying is minor
Section C Sign by the Sinle Parent

12. Invitation Letter

For Minor:

01. Application for Temporary Resident Visa [IMM 5257]
Employment What should we write
02. Custodianship Declaration (IMM5646)
The form says studying in canada...

03. Passport or Travel document
04. Digital Photo - FM

I read that the single parent needs to get affidavit or letter from the other parent when travelling the minor with one parent. In UAE, she had taken already the affidavit and form more than 6 years she have been sponsoring her daughter under her name. She also got US Multi-visa with her daughter without getting affidavit. Please can anybody advise please.
