I think, just when you already have your 2nd line does it mean someone has already been assigned to process your application. Subsequently will they do the background check. Anyway, you are almost there. Congrats!!!

Many people from India have been assigned to Warsaw. Yes it is normal.
Your application therefore will be processed at Warsaw instead of New Delhi

A number of applicants have reported such requests. I would say that they just misplaced the photos.

I have to travel to US for 2 months. Is this OK to travel outside India when the application is in process? And, do I have to inform CIC about this travel?

It's ok. Once you have an address in the USA, inform the VO by email.
