Reg. Name change - Middle and Last name changed Please help

I am on PR and I changed my name after getting PR and COPR so I have legal name change document also name has been changed in new PR, CIC mentioned COPR name never changes, also my new Indian passport has been updated with new name (name after marriage) . Now for the Question,4 a) and b) which name should I select as my COPR still has the same name as of my birth name (previous name) Also for Question 5 should I mention my previous name.
Q4 a) SHould it be my previous name like Simi Ketan Patel
Q4 b) new name after marriage as Simi Varun Shah
Q5. name at birth Simi Ketan Patel
Please note I have legal name change document as well from Canada, And my last name and Middle name has been changed.

Please advise, as I am unable to submit the form, it's giving me an error.

Q4a says, mention your name as per the latest immigration document. Therefore, your new name which appears on your PR card. The COPR is an older document which will not be rectified to reflect your new name.

Q4b would also carry your new name

Q5 would mention your old name & for reason, it would be 'Name at Birth'

I tried with that Q4 keeping new name same for a and b and Q5 as old name as name at birth but it is giving me an error as cannot be same as Full name
My last name and middle name has been changed and first name is as is..!

Can you please help
