Question 5 of citizenship application ?

Hello Seniors ,
I am using only one name since birth which i have mentioned in my citizenship application . should I need to mention that name again in the table which is provided in Question # 5 of citizenship application or not ?
Same question for my child citizenship application ?

Please advice .

Hi friend, I'm not a senior but I had the same question and when I searched, people have asked the question a few times before. The answer is that all the names need to be listed, so just list the same name again, mentioning it as "Name at birth". The same should go for your child as well.
