I have been married and divorced more than once. I’m now married to my husband in Canada and need to know whether in question 14 on IMM 5490 whether I need to only put my last husband’s name and divorce info or do I add the other one too. Only room for one on pdf form. If all previous marriages/divorce do I create additional page.
