Query: How do I prove that I do not have a USA refusal

Hi Everyone
I just received my Caips Gcms and it really shocked me as it says
The PA has not answered all questions truthfully- he did not declare his US refusal

But I have never applied for US visa. What you think is the problem and How can I overcome this reason as I want to reapply. Please help if anybody knows anything about this. Any kind of response will be helpful. Thanks.
