Problem uploading consent form while requesting ATIP notes

Hello,

This is my second time requesting notes but this time the consent form is not being uploaded. I tried in different browsers and different file format(Pdf, word, jpg, png) nothing worked. It does not upload the file. It tells me that I can't continue if I don't upload the form. I click add and click continue but it tells me again that I have to upload a consent form. Anyone else experiencing this?

Looks like it was a server issue last night. Today I was able to upload the form with no problems. If anyone ever experiences the same issue just wait until next day, it's a server issue.
