PR CARD

Have travelled on travel document issued on humantarian and compassionate grounds and was told at the airport to apply for PR Card. On first landing in July 2010 completed formalities for issue of PR card. Stayed in canada only for five days and did not receive PR card. A letter was received later indicating photo variations and required physical presence which was not possible. Now on landing on 10 Sept 2014 hve been told at the airport to apply for PR CARD. Need expert advise and guidance for completing the PR application. What do I mention in the Questions 19, 20, 21 AND 22 in PR Card application. It nowhere asks about travel document issued on humantarian and compassionate grounds. Can the reaons of absece be given on a separate sheet in answer to Question No.22 Travel History in PR Card application. And the reasons can be what has been stated while applying for travel document.

Please advise urgently so that PR card is applied in time.

Write a cover letter stating your humanitarian reasons for absence as well as explain that you were granted a travel document. Send a copy of the travel document as well.
