I am on a post-graduate work permit and would like to visit India. I am working with a company in a full-time position in Toronto. Which "form" should I fill so as I can come back to Canada after 15-20 days break.

Did you apply for a Temporary Resident Visa (TRV) after the work permit approval?
What is the expiry date of the visa stamp you have on your passport? The stamp you had when you applied for your study permit.
