Plz Help me!! Questions about application forms for the sponsored person

I'm filing the application to sponsor my common-law partner now (Outland) and i have some questions about the forms.
I really hope someone can help me with these. Thanks in advance!!!

1) Form IMM0008 (Generic application):
In Question 7 : Have you received your Certificat de Sélection du Québec (CSQ)?
If you checked “No”, they ask you to indicate the date when you applied for your CSQ.

Since we are applying from Quebec, we need to get approval by CIC first so we can apply CSQ. So we haven't applied for CSQ yet. Should I leave the date blank?

2) Form IMM5669 (schedule A: background/declaration)
In the guide, it says
This form must be completed by:
you, the principal applicant (the person I'm sponsoring).
your spouse or common-law partner (whether accompanying you to Canada or not)
So as a sponsor (the common-law partner to the sponsored person), do I need to fill up this form?

In the same form, question 6
d)Have you been refused refugee status, an immigrant of permanent resident visa(including CSQ) or application to the Provincial Nominee Program or visitor or temporary resident visa,, to Canada or any other country?

So if the sponsored person has been refused visitor's visa to the States, I guess a "Yes" should be checked for this question. Right?

1. Yes, leave it blank.

2. You don't need to fill out 5669. This is what we were told when I sponsored my wife, and we didn't have a problem.

3. Yes, your partner would need to report being refused a US visa.
