You should check the instructions because it should all be there. Sorry I don't have access to the forms easily so as to look and tell you, but when you look at your application package, look at the list of forms, and it will tell you the name and number and after that it will say the date of the form. That's it.

I tryed in google and found
IMM 5491 or IMM 5533
So I am confused
You only need one copy of IMM5532......filled by both Sponsor and Principal Applicant (i.e., wife)
IMM552 - Part A filled by Sponsor and Part B filled by PA.
If you are sponsoring your wife for PR in Canada......Do not use IMM5491 which is the 2016 version.
Use the Feb. 2018 version of the Spousal Sponsorship Checklist - IMM5533

But in any case, I just checked as well and it's very easy to follow, you should be able to find it. I'm only telling you this because the application is long and very specific, so you'll have to pay much attention and follow as it says on the website. Hope it hoes well
