It's your own personal checklist. Are you applying for a work permit? My fiance applied for a TRV and he was asked to do the same. They want you to create your own checklist of your documents because the document checklist is vague and general. I.E. in the document checklist it states "Proof of assets (land titles, car registration, etc)", so what you do is something like this:
❏ Application Form for principal applicant
❏ Application Form for Common-Law
❏ Current Passport
❏ Proof of Funds
❏ Bank Statement of Savings Account with Official Receipt (October 2010 - April 2011)
❏ Payslips (October 2010 - April 2011)
❏ Income Tax Returns (2010)
❏ Proof of Assets
❏ Car Registration with Official Receipt
❏ Driver’s License (photocopy)
❏ Makati House Land Title
❏ Certificate of Employment
❏ Company A
❏ Company B
❏ Proof of education
❏ Transcript of Records
❏ School diploma
That's just an example. Basically a detailed checklist of what you are submitting (it's for you, not for the embassy...so that you don't forget anything)
