I am hoping that someone can help with a relatively simple issue. I have been a PR for 11 years and have never left the country. My father who sponsored me lost all my documents about 10 years ago. I only have a photocopy of my passport photo and info page. Long story short, after sending an application in for a PR card, I received an email from a CPS stating that I am missing the page with the landing stamp. Aside from that, all information required from the passport is in the images I provided. I was told to provide a statutory declaration by CIC after inquiring about what I should do. Where do I get this statutory declaration form? I contacted a lawyer and I was told to bring an unsigned but otherwise filled out statutory declaration form. Where can I get this form? Can I just create it on Word?
I want to go on a vacation soon and will apply for citizenship when I come back.
