OPW mistake in name and last name

Hey guys,

I just received my OWP and I noticed one of my last names is written in "name" section instead of being written on the "last name section"

Example

Full name: Sarah Kendra Smith White

Now, the name and last name in the work permit are like the following

Name: Sarah Kendra Smith
Last name: White

So, how can I correct the mistake?

The name should be: Sarah Kendra
The last name: Smith White
