Open work permit - common law visa

Hey guys,

I'm applying for my common law visa sponsorship with my boyfriend and want to also apply for my open work permit.

The CIC website only lists IMM 5710 as a form that's necessary under sponsporship for common law spouse as quoted below:

"Your spouse or common-law partner in Canada can apply for an open work permit when they apply for permanent residence. They must include a completed Application to Change Conditions, Extend my Stay or Remain in Canada as a Worker form [IMM 5710] and pay the fees."

I see however more forms including one that needs to be signed by a notary for the declaration of commonlaw union if i just look at the work permit requirements.

Is this due to other information being provided by common law sponsorship documents or are all of these other forms mandatory as well?
