If a non custodial parent is applying for permanent residency, do the non accompany minors have to go through physicals?

Yes - they are supposed to go through a physical.

You can ask CIC to exclude them. However once this is done, they can never be sponsored to Canada by the non-custodial parent at any time in the future. CIC doesn't like excluding minor children this way and will typically make several requests for the minor children to go through the medical. Long story short, if you want to exclude minor children from the medical, expect the processing of your application to take longer.

And in order to exclude, immigration may request proof that you do not have joint custody / access to the minor children in order to make medicals happen and that you have requested that the other parent provides the children for medicals and the other parent has refused.

Immigration sees it to be in the best interest of the children to get the medicals so that they can later be sponsored in case something happens to the other parent, for example.
