Need some assistance in filling Work Permit form for Canada

Hi,

I reside in Sri Lanka and I have a small confusion. In our Passport, we do only have Other Names and Surname.

My other name : Kalugamuwe Muhandiramalage Shihaan Hussain
Surname : Irshad

But the form is requesting me to fill in Full Name (Family name) & Given Name

How do I fill this please? The form would be IMM-1295
