Mistake on Medical Forms

Hi All

Went for our medicals yesterday and spotted a mistake on the forms. The gender of one of the kids was noted incorrectly.

The DMP said it was a fairly routine mistake that he had seen on many forms and that he could easily correct it himself on the form. He did make the correction on the form.

Now the question is - should I also write separately to the VO to let them know that tehere was a mistake on the form and the DMP has made appropriate corrections?

VO is CHC London.

Has anyone had a similar experience?

Senior members please advise what action I should take in your view.

I have know other people who even got their children's photos swapped, i.e. son's photo was pasted by VO on daughter's and vice versa and when they took the forms to DMP, they simply swapped the pics. If DMP has already done it than i believe they are experienced in their field., although you'll get more views on that but i personally believe that there is no harm to write and email to CHC to let them know.
