missing documents - PR application

Hi all,
I just received email from CIC about missing documents for my PR application. quick question - what is the first thing listed?

Dear xxx,
This is in reference to your application for permanent residence in Canada. In order for us to continue the review of your application, we require the following documents:
- UCI number - my first and last name:
- Schedule 4 - Prov Nominees: A newly completed Schedule 4 Economic Classes – Provincial Nominees form
Please upload these documents to your MyCIC account by 2016/04/21.

Your UCI number will be present on the request letter itself (top left corner under the date).

yes I know, maybe didnt describe this clearly, they want 2 documents:
-one is schedule 4 which is clear what it is
-second one is not described, it's just my UCI number (hyphen) my first and last name - no idea what it might be...

They are only asking for one document and that's Schedule 4.

The first line (UCI & Name) is used to identify the person to whom this request is addressed.

This is not missing document request. Its rather a Schedule 4 request that all provincial nominees get. Fill it up and upload it in the corresponding placeholder.

The first line is your UCI number. They are not "asking" anything there.

yes, figured that out - i think it's to specify who they are requesting it for in case spouse/kid is included in the application

Schedule 4 was submitted with PNP app last year, however, the amount of money declared by me was enough for last year, but not enough for this year, that is why they wanted a new one
