Medicals Form - Clarification Required

We have received the medicals request.
It includes
1. A form with my husband's photograph, his details and CHC seal on it.
2. A form with my photo, my husband's details and CHC seal on it.
3. Another form with my details but no photo or seal on it.

Can anyone guide as what shud be done at this point cause we have only one month time to complete the medicals...

You should contact CHC, even this is specified in the letter accompanying the medical forms, that if there is any thing wrong, do contact the CHC as earily as possible.

Contact them at an earliest and they wull send u updated forms. after receiving u have to destroy the forms u have already recd.
