I have two children (8 years old and 4 years old): The older child is one of the three applicants (me, my spouse, and my older child) for the permanent residence, and the younger child is a canadian citizen. I'm waiting for the medical check request. Does my younger child also have to get the medical check? Thank you so much for your replies.

No. I believe you have to declare your citizen child(ren) in the PR application, but only as "non-accompanying", for obvious reason. Thus medical not required...
