malapit na ito,,, kse my our medical result has forwarded by st. lukes april 25, 2014 lng eh but the NBI renewed and the payment for RPRF and the copy has been received by CPCV april 10, 2014....talaga lng nman ang e-cas ay di up to date...the important is the communication from the CPCV, The letter coming from them, the instructions and the issuance. di po ba?

if u don't mind me asking, kelan nag submit yung nag sponsor sa inyo? Month n year of ok mag back read ka lang Dito and you'll just need to bring the form ng medical wala talagang request letter

Saan makakakuha ng Medical Report form (IMM 1017E)

In my case tumawag ako immigration and request the form and they send it to my email and snail mail too. Better that way kasi sa MR andun UCI. At first naghintay ako it takes a month kakahintay wala sila pinapadala kaya tumawag ako at pinadala nga.

hello po,, I would like to ask those who have received a letter for additional documents what does these mean:

My father (LC2) is required to submit the following:
1. pre and post natal care documents for daughter and mother
2. joint affidavit of 2 witnesses
3. affidavit of delayed registration
