As for required documentation, you need to read the application guide and document checklist. Note that there are 2 stages to the application - the first one to centralized intake does not require that you submit all the documents. The full application to your local visa office does, and may have specific instructions regarding the types of documents you have to submit. Can't tell you which local visa office instructions apply to you without knowing where you're from...
