Indian Passport Surrender

Hello,

I have a query while filling online application form for surrendering Indian Passport.
Query: What goes exactly inside the field Place of Birth (Village/Town/City) .

My current Indian passport has City, State mentioned so what should I enter on the form. Should it be exactly like as passport or just city. Also if I try to enter both City and State, the online form field length is not enough to capture both.
