If your new Doctor has a copy of your child's records I don't see a problem with them completing the immunization history. Just be absolutely sure the Dates are Correct. I have spent too many hours trying to sort them out. Be sure Day/Month/Year cannot be confused as some countries write Month/Day/Year and it is hard to know exactly what an example like 10/11/12 might be.

Can we ask General Practitioner (GP) Doctor to certify the immunization history or it should be done a Paediatrician (Child Specialist)? And also, I am planning to scan the form and fill it electronically. Is that OK?

We are landing next month but will be staying in Canada only for 15 days, get our paperwork and come back to Dubai and move permanently next year August.
We have a 5 year old son and we have received the immunization paper for Canada Immigration. Do we still need to do this beore landing?

Would be grateful if you could throw some light on this. Where in Dubai can we get it done and who is considered an authorised medical professional to attest this.

i would suggest you take the forms to the same person or office where the immunizations were done. In Canada, and many other countries, immunizations are done by nurses and family doctors so a specialist is not required to fill out the forms. If you have moved, take your child's immunization records with you to your next appointment at the new Doctor or Nurses Clinic and have them complete the forms.

immunization required for kids admission to cnadain school, so u can bring it when finally shifting to Canada not now..

i have a question though...

i already have a record of immunization for my daughter, signed by her pediatrician and printed on the hospital (the one he is affiliated with) letterhead and with a dry seal affixed. do i still need to download this form and have the pediatrician sign it or the document that i have will do?

Hi! Did your copy suffice? We have the same situation. We're from the Philippines and during our medical examination, we were given vaccination for MMR. Along with it is a certification where it indicated the dose of the vacc and the date. Also, the header is of the St. Luke. There was also the name of the doctor who administered the dosage and her license #. So, do I have to download the form and make a pediatrician sign or will what we have will suffice? Thanks in advance!

Hi All,

I have noticed that in some health related professions, immunization record for adults is required, which includes
A satisfactory medical exam.
An up-to-date immunization record including tetanus and diphtheria, polio, measles, mumps, rubella, varicella (chicken pox), Hepatitis B (incl. Hep B titre within the last 24 months) and TB skin test.

It is also required for getting traing in health related fields as well.

Anyone has idea how to arrange for such an immunisation record for Adults especially in Gulf.

Kindly advise......

If anyone hasn't got the above form from the CHC while receiving their PRVs+COPR, and have kid/s under 16, plz click the link above to download that form.

U'd need this form to be filled, signed & stamped by your doctor, from your homeland, before departure for landing. It's required for the kid's School Admission in Canada.

And that's a strictly enforced regulation here.

The form is on this site because CIC doesn't provide it.
One of the old forum members got it from CHC London with his COPR.
And
Thanks to Qorax, who shared it through this site, as it is difficult to mail everyone requesting.
Why CIC is not sending... is a question to which i have no answer.

My son is 1 year old and we are not giving him any vaccines. Would he still be allowed to enter without any vaccination proof whenever we are finally approved for PR?
I am most likely planning on doing home school for him when he is older or possibly private school, but are vaccines necessary for immigration?
