IMM5406 HELP

My imm5406 form has been returned and asked to be updated.
I am the principal applicant. I've sent one for myself and one for my husband. Should I have only sent one? Should I send two? Do I put applicant or spouse on my husband's form?
No explanation from CIC except that they want it sent again. They've also asked for police checks to be resent and these are still well within their year's validity.

Any help much appreciated as always.

You each need to have your own and are each the applicant when filling out your own form. Did you list all parents, siblings, kids properly?
